Community Health Needs Assessment

The Community Health Needs Assessment (CHNA) report provides information on a study of the health status and health needs of residents in the communities served by Redeemer Health.

The purpose of the needs assessment is to identify and prioritize the community's health needs to guide Redeemer Health's strategic decision-making and planning, so that resources are devoted to services that are most necessary. In order to best understand the community's needs, Redeemer Health studied market data and collected regional intelligence, discussed needs and planning with key stakeholders, and distributed its own survey to community households to gauge current needs and ability to access services.

It is important to note that our survey and action planning pertain to the region immediately surrounding Holy Redeemer Hospital in Meadowbrook, Pa., however, some services identified as critical are located in a larger region extending beyond our primary service area of eastern Montgomery County, lower Bucks County, and Northeast Philadelphia. 

The Patient Protection and Affordable Care Act of 2010 requires nonprofit, tax-exempt hospitals to assess the health care needs of their communities every three years and to adopt an implementation strategy to address the needs identified. Each hospital must make its Community Health Needs Assessment (CHNA) widely available to the public. Below are links to the current needs assessment, and assessments performed in recent years.
