How Special Recovery at Home Works
As part of your initial assessment, a member of Redeemer Health's Home Care team will visit you at home or in the hospital to determine your needs, which may include skilled care, such as nursing or therapy, and talk with you about your health goals and concerns.
Based on this initial assessment, we will work with you, your loved ones, and your doctor to develop a step-by-step plan of care. This program is designed to allow you to rehabilitate, recuperate, and continue living independently as you work toward your health goals.
Redeemer Health's In-Home Therapy
Therapists evaluate your needs and develop a customized program to fit your goals and abilities. Assessing your needs within your home provides greater insight into your needs within your own environment.
Redeemer Health's home rehab services include:
