We provide primary care and internal medicine to individuals aged 16 and over. Our focus is to provide personal and continuing care for individuals and families, with services that include:

- Referrals to specialists

Patient Portal
At Holland Medical Associates, we want you to be empowered about your own health. That’s why we offer a Patient Portal—a secure place where you can view your test results and health records, as well as schedule appointments.

If you’re a new patient or haven’t received your Patient Portal sign-in information yet, be sure to get it at your next appointment.
Prescriptions
For prescription refills, please submit your request through the Patient Portal or call our office and follow the telephone prompts. Please allow at least 24 hours for the prescription to be called in. You’ll need the following information:
- Your name
- Your date of birth
- Your phone number
- Name of medication, dosage (30- or 90-day supply)
- Pharmacy name and number
Referrals
For referrals, please submit your request through the Patient Portal or call our office and follow the telephone prompts. Please allow at least 24 hours for the referral to be completed. You'll need the following information:
- Your full name
- Your date of birth
- Name of specialist
- Provider NPI number
- Phone number for specialist
- Patient diagnosis code
- Date/time of appointment
