Patient Portal
At Holy Redeemer Family Health at CARDONE, we want you to be empowered about your own health. That’s why we offer a Patient Portal – a secure place where you can view your test results and health records, as well as schedule appointments.

If you’re a current patient of At Holy Redeemer Family Health at CARDONE and have received your patient portal sign-in information, sign on now.

If you’re a new patient or haven’t received your Patient Portal logon information yet, be sure to get it at your next appointment.

Prescriptions
For prescription refills, please submit your request through the Patient Portal or call our office and follow the telephone prompts. You may also request refills through your pharmacy. Please allow at least 24 hours for the prescription to be called in. You’ll need the following information:

• Your full name
• Your date of birth
• Your phone number
• Name of medication and dosage (30- or 90-day supply)
• Pharmacy name and number (or mail order info)

For referrals, please submit your request through the Patient Portal or call our office and follow the telephone prompts. Please allow at least 24 hours for the referral to be completed. You'll need the following information:

• Your full name
• Your date of birth
• Name of specialist
• Provider NPI number
• Phone number for specialist
• Patient diagnosis code
• Date/time of appointment
