How to Contact Us After Hours: If you have a question or concern after hours, please call the office at 215-357-4066. You will be directed to our answering service number and you can have a physician on-call paged. In the event of a medical emergency, please call 911.

Patient Portal
At Redeemer Health Primary Care at Warminster, we want you to be empowered about your own health. That’s why we offer a Patient Portal – a secure place where you can view your test results and health records, as well as schedule appointments.
If you’re a current patient here and have received your patient portal sign-in information, sign on now.
If you’re a new patient or haven’t received your Patient Portal logon information yet, be sure to get it at your next appointment.
Prescriptions
For prescription refills, please submit your request through the Patient Portal or call our office and follow the telephone prompts. You may also request refills through your pharmacy. Please allow at least 24 hours for the prescriptions to be called in. You will need the following information:
- Your full name
- Your date of birth
- Your phone number
- Name of medication and dosage (30- or 90-day supply)
- Pharmacy name and number (or mail order info)

- Name of specialist
- Provider NPI number
- Phone number for specialist
- Patient diagnosis code
- Date/time of appointment
