At Redeemer Health Family Health Center at CARDONE, our mission is to provide the highest quality medical care, education, and wellness programs to patients of all ages and conditions. Services are available for employees of CARDONE Industries, as well as residents of the surrounding community.

As part of Redeemer Health Physician & Ambulatory Services, Redeemer Health Family Health Center at CARDONE has access to an entire range of healthcare and health-related services for every stage of life, including a full-service hospital, home care and hospice services, adult nursing and retirement communities, and low income and transitional housing programs for those in need.
Patient Portal
At Redeemer Health Family Health at CARDONE, we want you to be empowered about your own health. That’s why we offer a Patient Portal – a secure place where you can view your test results and health records, as well as schedule appointments.
If you’re a current patient of Redeemer Health Family Health at CARDONE and have received your patient portal sign-in information, sign on now.
If you’re a new patient or haven’t received your Patient Portal logon information yet, be sure to get it at your next appointment.
Prescriptions
For prescription refills, please submit your request through the Patient Portal or call our office and follow the telephone prompts. You may also request refills through your pharmacy. Please allow at least 24 hours for the prescription to be called in. You’ll need the following information:
- Your full name
- Your date of birth
- Your phone number
- Name of medication and dosage (30- or 90-day supply)
- Pharmacy name and number (or mail order info)
Referrals
For referrals, please submit your request through the Patient Portal or call our office and follow the telephone prompts. Please allow at least 24 hours for the referral to be completed. You'll need the following information:
- Your full name
- Your date of birth
- Name of specialist
- Provider NPI number
- Phone number for specialist
- Patient diagnosis code
- Date/time of appointment
